I would think that anyone who had a PSA above 20 would probably have advanced PCa. That is the cancer may be outside the prostate. A Gleason of 4+3 and any number above this may be considered advanced. If a bone scan shows metastases, it would definitely be advanced. If the PSA is 10 or less and the Gleason is 3+3 or less it may be classifed as a moderate disease. I wish you all the bestAubrey Pilgrim, DC (Ret.) As it turns out he was a sleaze who would have had me laying on his DaVinci Robot in 30 seconds if he could have.Given all the gradations and forms of Prostate Cancer I don't understand he kept using the term "Advanced Prostate Cancer". Guest guest Posted November 30, 2007 Report Share Posted November 30, 2007 Sy, I would consider " advanced prostate cancer " as being recurring disease after failed initial treatment, or when at diagnosis appropriate testing/imaging determined that the cancer has extended beyond the gland to lymph nodes or seminal vesicles or known to have metastasized to bone. Dr. Judd Moul of Duke University Medical Center has remarked that the term should even be broadened to include patients who, despite presenting with low-grade prostate cancer, show evidence of high risk of progression, even life threatening. I can only presume that he sees such cases as requiring the more intensive observation and treatment of such patients as would also concern the Medical Oncologist in treating advanced prostate cancer. You are wise to question his advice. That is why you will always hear about " 2nd opinions " . Personally, I would go for 3rd 4th and 5th opinions if my life and future quality of life is at stake. Since my experience with a lot of incompetent advice, I have changed doctors a lot and included on my advice team a radiation oncologist, and a couple of medical oncologists. In your case, I would not ignore a radiation specialist who can give you an opinion about seeds. Guest guest Posted December 2, 2007 Report Share Posted December 2, 2007 My doctors have told me that their definition of advanced prostate cancer may vary but it is typically viewed as cancer that is no longer confined to the prostate capsule. I was told that my pT3b was advanced because the front line treatment failed to remove all of the disease. I was urged to not hesitate and take further action after LRP to attempt to keep the cancer from relapse. I was able to get to an undetectable PSA and still have that blessing.
