Christal T.,

Could this be a VP shunt vs. a PV?

If it is VP shunt not sure about a web site but we do see a fair number of

these patients....usually these patients have been premature infants who have

had some type of intraventricular bleed and they may then develope

posthemorrhagic hydrocephalus soon after. Usually they identify the condition

while the baby is still in the hospital. The VP shunt is placed to drain fluid

from brain to the peritoneal area where it is absorbed, hense the name " VP "

shunt.

When we, EMS, see them they are usually because they have their shunt blocked

from a FB or infection. They are usually lethargic, irritable, febrile,

vomiting, and may have a tense fontanelle. As I understand it there isn't much

we can do about the problem as it usually requires removal of the FB/infection,

but obviously we will need to provide supportive care. Here is where one has

the obligatory comment about " follow you protocals " . You may find info if you do

a search on the web or there is a Pediatric Emergency Medicine book (I think it

is red and white)you may be able to find at a medical book store. It is an

excellent reference for many topics that some of our EMS books either do not

cover or do not cover well.

Hope it helps.

Found some good info on this issue. Go to askjeeves.com

type in the question " what is ventriculoperitoneal shunt? " You will get

three sources that seems to be pretty comprehensive, sorry could not find

specific web addresses to those sources. Hope this helps.

The only thing I can add to the other posts is that if you transport the

child, try not to flew the neck to the extreme away toward the opposite side

(I don't know why you ever would, but I was told not to when I asked about

VP shunts), and avoid pressing on the shunt either with your hand or with a

c-collar.

[texasems-L] Pediatrics

Hey group,

I encountered a pedi today with history of a PV (possibly peavey) shunt from

hydrocephalus. Does anyone know of a good website I could research for

info.? Since he is a resident of the town in which I live, I will probably

encounter him again and would like to be further prepared.

Thanks so much for your constant support,

Christal T.

Just got back from the PEPP rollout. In the Children with Special Health

Care Needs sections VP shunts are discussed. When you hear of a PEPP going

on in your area- take it, learn it, do it...
