Candace-

diagnosed me with pudendal neuralgia too. However, my situation is

different. She believes that my tilted pelvis is causing an entrapped nerve.

She seemed fairly hopeful that PT would help me, and then when I saw ,

she said that I have a 98% chance of getting 100% better. She said I was a

" classic " case of a biomechanical problem causing vulvodynia.

I just started PT here in St. Louis. However, this PT focuses on

biofeedback. She doesn't do the manual manipulations that did. I'm

hopeful that this will work, but I have a feeling that I'm going to need to

get my pelvis in position if I want to get out of pain. Biofeedback won't do

that.

Hi ,

Just finding out about it myself. Its pudendal, by the

way. Its a nerve in the pelvic floor region,

supposedly one cause of vulvar pain when it is damaged

or irritated or in some way not behaving correctly.

said that if it is just " stuck " and we can

release it, she hopes to be able to reduce/elimnate my

pain, but she cautions that if it is " damaged " then is

unlikely any PT would correct it. She said it is the

toughest of all to deal with in the world of vulvar

pain/ nerve conditions. This all " fits " for me in 2

ways. First, this nerve is connected to a nerve in my

lower abdomen which I believe has been the source of

my pelvic pain for 17 months. Second, since the pelvic

pain started directly after my cervical surgery, I

think the lower part of the nerve could have been

damaged then, with the vulvar pain manifesting 10

months later. In effect, I'm wondering if while I was

sedated the doctor, whom I really like and trust, by

the way, may have " speculumed " me into this condition.

Who knows?

I only saw once and twice-all in one weekend. I flew out to

Philidelphia for the weekend, specifically to see them. My aunt lives out

there so it was also an excuse to see her.

The worst spot that hurt on me was my piriformis. That is a little muscle in

the butt. She pushed on it externally, and it killed. One side hurt worse

than the other side, but they both were tender. She said that the pudendal

nerve runs through this muscle. She explained what may have caused this

entrappment. First, I have scoliosis in my lower back which caused my pelvis

to tilt. Then, I developed IBS about 7 years ago which causing bad cramping,

which further pulls my pelvis upwards. Also, four years ago, I quit

competitive swimming after 10 years of intense training. Swimming doesn't

work the piriformis at all, but rather develops all hte muscles around it.

This also could have contributed to the problem.

Interesting though, that when I started PT last week, the new PT didn't find

any of my tender spots.

doesn't do any biofeedback. I rally hope that this biofeeback works.

Are you going to see regularily???

Hi Christy,

Thanks for the details. How many times did you see

before moving? I wonder why was so

cautious about being optomistic, still do, can't wait

to see what says.

Did the PT hurt? When pressed on " the " nerve, it

hurt, but not screamingly, and I just don't get how my

whole vulva hurts because of this one odd left-sided

nerve that doesn't on its " pressing site " really cause

terrible pain. I was so distracted by my abnormal

bleeding and her questions, I did not get many of my

own questions in at all.

She said my type was quite rare, or did I mention

that? don't know how it all relates. If anyone out

there does I'd love to hear about it.

I agree about the biofeedback, but hope you find an

answer soon.

If you injure a part of your body, the surrounding areas frequently become very

painful. For example I dropped something on my foot and the whole foot swelled

up. Also it could be referred pain.

Christy and Ora,

Thanks for your input. Ora, I just have a hard time

" getting " referred pain, although I understand it

intellectually. My left ear has been painful for

months(interestingly enough, that pain came on with

the vulvar pain...) An EMT found not a thing wrong

with my ear, the best he could say was TMJ and assumed

I have headaches, which I do not. And my jaw does not

hurt. I have a hard time beleiving THAT referred pain

diagnosis. Especially since the best he could do was

say " advil and soft food " .!

Christy, I'm not surprised by the very painful point

she found, but that is my question, my " point " did not

kill, just hurt. Cool about the swimming, are you

still physically active? I still ride a little, but

have not exercised for months, very unlike me.

I have my very first appt with August 15, so I

have no idea how often I will be going yet.

wrote a script out for 16 sessions, but did not

specify the frequency.

Thanks for the explanation Candace. I hope PT WILL help correct your

problem. I am due to go back to my PT tomorrow and I only began yesterday

trying to " relax " and " release " my pelvic floor muscles. Something about

sitting or laying and " relaxing " is just SO hard to do for me but I will try.
