Right...BUT..if the boy's problem is not thyroid and it is something else that endos deal with there may be something they know.... As for the ACTH being low normal and the am cortisol being low, this is why they are thinking secondary. However, ACTH fluctuates MORE than TSH based on stress level. ALSO, if there is a different condition going on, and the body does NOT want increased cortisol, that can account for this as well. This is going to be a fight with these docs...and it i a shame....but you need to focus on getting to the bottom of the B12 issue and try the acth stim IF you cannot first get to the bottom of the B12 issue. The cortisol may simply be low due to another chronic condition. In itself it will cause symptoms...but the body will slow down to compansate.
