Guest guest Posted June 24, 2003 Report Share Posted June 24, 2003 Dear Group- We , like other inpatient rehab facilities have been using FIM, and now the IRF-PAI, for a number of years. Despite over 15 years experience with the FIM we recently seem to be having some difficulty with tracking the variable performance over that all important first 72 hours. So, I have a few questions.... Do you have a single ( or several) persons score the entire instrument, or does each discipline contribute specific section(s) ? If nursing is involved, do you train and " credential " both the aide staff ( who do most of the direct patient care) and the RN/LPN? How do you record the variability over the first ( and last) 72 hour scoring period? And over that last 72 hours, since we have to take the lowest score in both instances.... Do you discuss the FIM/IRF-PAI scores in patient evaluation conference, as a way of finding out the lowest score in that 72 hours? Any other insights you can provide about training would be helpful. We have had staff go to Buffalo to train with UDS, we've had UDS down here to tain trainers, and now we have one person who coordinates and does most of the training. Therapists do very well on the crendentialing but other disciplines struggle for us.... Thanks very much for your insights in advance.
