Any doctor can sign the form for you. Pick up the form at any Dept of Transportation office (or often the State DOT offices have them online so you can print them off). The rules to qualify are printed right on the form. It simply requires a doctor's signature, and then you take the form back into the the DOT office and that's it. No examination required.
