1st follow up visit w/ neph.

Hello all,

I just met with my neph after one month on meds (40mg

prednisone, 10mg prinivil, 20mg pepcid, and fish oil). Results seem

to be somewhat promising----protein went from 1.6g to 1.3 (not much

of a difference but at least it decreased) and creatinine clearance

increased from 87 to 148 (this seemed very surprising). My bp is

still high though averaging 140/82 before working out and dropping to

125/72 after working out (I have noticed some pain in my knees and

slight cramping in my legs that I did not previously experience).

I appreciate any feedback on my results. I am hoping to start

contributing to this group in the near future instead of just asking

questions. Right now I am just trying to educate myself about this

disease and absorb as much as possible from more experienced people

like yourself.

Nice to hear from you again. Well done on the slight decrease if your

protein. What did your Neph attribute this to? I wonder if any of the

medication you are on could be causing your cramps? Anyone else in the

group take 's meds, and suffer the same?

Yes your BP is a bit elevated, but I have had episodes of very high,

then very low. Trials with Altace were a big flop, but since then, I

have maintained a 90/65 which is a tad low at the moment.

I believe my neph attributed the decrease in

protein to the prednisone but when you treat the

disease with a combination of drugs it is hard to tell

which one is responsible (I am just grateful for the

slight improvement---it is the little victories along

the way that add up). I feel that the cramping and

muscle aching is a result of the prednisone also---not

sure?

Hi .

If I'm reading you right, you started all those at the same time a month

ago? This is not uncommon, but one problem with it is that if anything

works, there is no possible way to know what's doing what. Protein be down a

little is good, while creatinine being up may or may not be that

significant. For one thing, with ACE inhibitors (prinivil), it's common for

serum creatinine to increase a bit - but it's not considered to be

significant unless it's by more than 30% - a good trade-off for the overall

renoprotective effect, as well as the anti-proteinuric effect. Secondly,

there could be other factors that cause it to be a bit higher, like a bit of

dehydration, for example.

With BP, it's not what it is after any given activity that counts. What

matters is the average resting blood pressure. Yours seems only mildly

elevated, and only as far as your systolic goes. It may be worthwhile

discussing with your doctor how you can lower it more. It doesn't

necessarily have to a medication (although a tiny dose of a common diuretic

combines very well with prinivil for more effect, and is usually very

tolerable). If you eat a lot of salty food, cutting back on it and

abandoning the use of table salt will likely do the same thing as taking a

diuretic. Aerobic-like exercise that works out the cardiovascular system can

also help. With IgAN, high BP tends to creep up on you over time.
