documentation of pain

Just curious how other JCAHO facilities are handling the documentation

of pain assessment/reassessment in acute and OP settings. Please answer

the following questions if you will:

1. Is pain scale used and documented at every single visit?

2. If it is not used at every single visit, does your facility have a

policy to address when it may not be used (ie, if there is no pain ID'd

at eval...or pain documentation by exception if nothing ID'd at eval).

Thanks...I appreciate any feedback!
