In Kansas, we would go ahead and notify the doctor via phone, then write a

separate D/C order to provide documentation on the chart. The initial order

should cover for the eval, but the separate order just bolsters the lack of

need for PT services.

We do not provide a seperate order as the eval order is sufficient for

providing the eval, then we notify the MD by phone to let them know no

further need for therapy. The less orders to track and less paperwork the

better.

The way our agency handles this is that the initial order is the evaluation

order. So if we determine there is no need for therapy, we write for no

further need and discharge. You are correct in thinking the inital visit

should be covered under the original order.

The pt I am referring to was only referred to PT and after performing an

evaluation, I determined that there was no skilled need for PT. I

completed an open OASIS and eval. I am now hearing from my agency that a

d/c OASIS and 485 need to be generated in order to bill for the visit?

Is that accurate or is there another way to bill for an eval only

without having to go through all that paperwork?

I'm somewhat confused by the scenario you report. Is this patient not

admitted to home health services (in other words will she receive

another qualifying disciplines service)? If PT determined there was no

need for services and no other qualifying service will continue, it

would seem the patient would be a non-admit. In this scenario another

order would not be necessary. It would require documentation of

consultation with the referring physician. If the patient will be

continued by another discipline, the order for the one visit of PT can

be placed on the 485 (plan of care). I am not aware of any reason for

obtaining an order for an evaluation only, but maybe it is relative to

an agency policy/procedure or an intermediary specific decision. Jimmie

At our agency we would also complete a 485. However, we would not complete a

DC Oasis as there is no care, no visits planned nor provided after the admit

visit.
