Medicare 700/701 Forms

To the Group:

Can anyone please clarify whether filling out the Medicare 700/701

forms is necessary in an outpatient physical therapy clinic or is it

just as good to have a signed plan of care from the physician?

Can you survive a Medicare audit without completing the 700/701 forms?

In the Medicare Outpatient Manual it does not state it is a necessity

to use these forms. I called a Medicare representative to ask the

question and this person stated, " No " , and was not even to familiar

with these forms.

Several PT's I know do NOT fill these forms out and several do.

In summary, for a start-up physical therapy practice, what would you

recommend? We want to start on the right path and be compliant with

whatever systems are in place.

We have used these forms for the past 12 years...as a part of our

introduction to the Medicare programat the time it was suggested that we use

these, get them signed and keep them inthe file as a physican's formal

agreement to the plan of care. Many have chosen to ignore them..many don't

even know they exist....a pure product of the splintered organization that is

taking care of our seniors and disabled...anyway...it is an easy form and a

sure fire way of proving the MD's approval...after all it is THIER form.

What ever you use a plan of care of your own choice would no doubt suffice as

well.. good luck and welcome to the " madness. "

, sorry I didn't get to finish my last e-mail, it got cut off. I was

saying that I would also like to see your work since you already went through

a Medicare audit without any problems. I met you back a few months ago

actually I believe on a job interview. I am now the program coordinator and

physical therapy CORF clinic in Hollywood that is just opening so I could use

the guidance. Thanks in advance.
