You will have to sign a release for your medical records. On that release

you will have to state what you are requesting---discharge summary, doctors

records, tests and radiology records, etc. Most hospitals require payment

for each copy. That can be very expensive. Plus with the years it has been,

they are probably on microfilm and will take time to research them.
