PERMISSION TO RE-POST AND FORWARD TO GROUPS

Many of you are already familiar with AFFTER's Questionnaire that began in

1999. The purpose of this survey is to evaluate the symptoms and

characteristics that people with fibromyalgia share. Thank you to the

hundreds of people who have taken the time to fill out the written

questionnaire and return it to our office! You may have seen some

preliminary results we had published in AFFTER's newsletter (Volume 3 - Fall

2000), which was based on about 100 written surveys.

We now have made this questionnaire available on-line in order to reach more

people and make it easier to use. If you have already submitted the written

version, or if you aren't sure, please complete the on-line version anyway.

We have the capability of sorting that out and we can use your most current

survey for a follow up study.

We have also created a survey for use as a control group, so please forward

this email to your friends and family members who do NOT have fibromyalgia

as well as those who do. Here are the two links that are self explanatory.
