I copied a HIPAA form from another local doctor, and I have it sitting on a small table in my office. The first time I see a patient, I have them sign a form that states that they have been given a chance to review the HIPAA statement, and also gives me permission to bill their insurance ('Signature on File'). I then import that into their EMR, with a copy of their insurance card. The form just says they have read it. Along those lines, how do you organize your papers in general that you receive from labs, consultants, PT/OT, etc etc. ? Where does the copy of the patient's insurance card, license, demographics go? One insurance plan told us we had to even have a copy in each exam room or they wouldn’t approve our site. Ours is on our web site and we have it at the bottom of the pile of paperwork that we have new patients fill out. I think I’ve actually had 2 patients (out of 1300 new patients in the past 3 years) read it. We condensed ours from 4 pages ( the AMA has one for download available) to 2 pages (1 page front and back copied at Staples for 12 cents/page on colored paper). Guest guest Posted February 15, 2008 Report Share Posted February 15, 2008 Our EMR (e-MDs) has categories already set up by specialty. Admin forms go in “administration”, labs go in “labs”, etc. Insurance cards and driver’s licenses are scanned on a card scanner at the front desk and are saved elsewhere so that you see them when you open the patient’s demographics. We can print a “chart cover” if necessary, which will print a copy of the insurance card. The only down side is that it will currently only scan a 2-sided cards, and we’re seeing a lot of 4-sided cards, so we pick and choose which 2 sides we want. The P/T doc that is in our office just converted last week from paper to our system, and I’m happy to report that he very rarely is referring back to his paper charts because we transferred all of the pertinent data (allergies, meds, PMH, etc) into the chart and scanned his last 3 labs and progress notes into DocMan. It is MUCH easier to find things in the EMR than it ever was in his paper charts, and anyone in the office has instant access. Pratt Office Manager Oak Tree Internal Medicine P.C. Guest guest Posted February 15, 2008 Report Share Posted February 15, 2008 When I first opened my practice, I went to the two nearest hospitals and got admit forms, and then to the nearest "Big MD" offices hear them and got new patient forms. All these had been vetted (personal joke) by a subset of local lawyers and filled all legal requirements. Then I gave them to a non-medical friend to evlauate for readability. Then I consolitated duplicate statements, put all permissions on one sheet, including the last permission to use a copy of their signiture for an original. I added a review of systems the resulting four pages goes to all new patients; I scan them into the EMR along with a copy of their insurance card, and copy the information into the billing system: then I can see them. In general, this takes 15 minutes because they have to read them. I have a copy of my "Big Privacy Policy" in the waiting room to threaten them with if they want it. One nice side effect is that it stops all the casual drop in people who ask me to "just take a peek at this problem and see if they really need to see a doctor" because it is illegal for me to do that without the needed paperwork.
