I used an M.D. but Prometheus may accept samples from other sources. I don't know. The name of the lab is: Prometheus Laboratories, www.prometheus-labs.com 5739 Pacific Center Blvd. San Diego, CA 92121 (888) 423-5227 The test is called " CeliaGENE " -- " A genetics-based test that identifies HLA DQ2/DQ8 haplotypes associated with celiac disease " The test cost $440 and insurance seems to be covering half of the cost in my case..
