New (06/09) Medicaid Review form

I just got our yellow Medicaid Review form. This one is significantly longer

than the one we got last year. Last year, the form only asked about income and

assets for the covered person. This year, it asks for " proof of all gross

income....received by all members of your household. " Do I understand correctly

that I have to supply information related to income received by my husband and

myself and not just my child now? Thanks!
