Hello All,I saw a client yesterday. Her baby is 3 1/2 months old. I am the 4th IBCLC she has seen.She had oversupply from the beginning. Her baby has a posterior TT - which she immediately said she would not release.Her baby has been refusing breast. She feels like she has to have very full breasts or he will not nurse. He only nurses in specific positions in a darkened room. This is her 2nd baby - she had a similar situation with her first.This baby was very fussy yesterday so I could not get a sustained suck assessment but what I got was not strong, nor was it terribly weak.He did take in 4.6b ounces in the feeding session that was off and on, distracted - about 15 or so minutes.I gave her some strengthening exercises. I sent her info on TT even though she is against release.She is returning to work in August. She says he really refuses breast when she has been away (minimal but it has been a couple of times full days) and he has had a few bottles.This mom knows how to pump - how to manage her supply. I recommended also bodywork - osteopathy, CST, chiropractic.She feels I did not give her enough perspective on her situation and she has requested a 50% refund. This part is just my own stuff but I am adding it - she lives pretty far from me - it took over an hour to get to her - I usually do not use a babysitter but I did as I knew it would take longer - she was recommended to me by a mutual acquaintance.She did say she would not push about the money but I am pretty upset.When she tried to justify by saying I was the 4th LC I asked if any of them had returned half the money - she said no because they had given her some perspective.Help!
