This is what I include; Intake sheet Consent for visit and acknowledgement of HIPPARCHUS Copy of HIPP (thanks to Liz Brook's cd) welcome letter with detail of mission and fees Consultation and history Assessment Care plan Pumping & storage Tracking sheet for feeds, pee & poo Who growth chart Blank note sheet for follow up calls Billing sheets 1 for me; 2 for mom (thanks to pat Lindsey Reasources(LLLI, Ameda pump retailer) My business card Would you include anything else?
