Sample Accomodation Letter???

Hello ladies,

I had a mom contact me today begging for a letter requesting accomodations be

made for her to nurse or pump for her brand new baby while she is taking a

standardized test that is 8 hours long. Her baby will be 6 weeks at the time of

the test. There is only one 30 minute break during the day and she is wanting to

request an additional break to either pump or feed in the morning and one in the

afternoon. The kicker is she has to turn in the letter by this Friday in order

to have it considered and she just called me today.

Does anyone have a letter that requests similar accomodations (from jury duty, a

job, custody situations, etc.) that they might be willing to share? I would

greatly appreciate it. I know it has to exist somewhere and seeing as I am

really pressed for time I'd rather not completely reinvent the wheel. Thank you
