Advice re: HIV infection

Dear Physicians of MGIMS,

I have a lady patient of mine, who had been diagnosed as HIV positive 5

years ago. About 2 years ago, her CD4 count dipped to 450 and viral count

exceeded 1 lakh units, so she was started on Duovir-N (Lamivudine +

Zidovudine + Nevirapine).

Since then, her viral load has come down progressively and is now around

4500. However, her CD4 is also down to 145.

Could anyone tell me if this requires a change in drug regimen or a gap of

no-drugs for some period before restarting? Or is this drop in CD4 nothing

to worry about, as long as the viral load is down?
