if you are coming to nagpur,do not hesitate to ask for any assistance

Dear Richa,

Apparently, there is one Dr Rai in BHEL (Kasturba Hospital), Bhopal, who does

total knee replacements. I am told that his results are good.

Dear Richa,

Just a few considerations.

1. The starting point: Pain relief is the primary goal and not

mobility. Given the bilateral osteoarthrosis, she may need TKR in the

other knee at a later date.

2. Rather than the actual implant, search for a good surgeon. Stick

to specialists doing total knees / hips predominantly. Avoid cowboy

orthopods. Enquire about the number of TKRs he does per annum,

success rates, mortality and morbidity rates, and infection rates;

and to lesser extent his pocket rates. Infection can be devastating

and therefore the setup and theatre needs to be modern.

3. Given her medical history we can expect a slightly prologed

recuperation. Postop physiotherapy is important and a good team is

needed. I would use the surgeon's team as they would be familiar with

his protocols. This can be done on outpatient basis (but time

consuming for the family attenders). So enquire how long she will

need to stay locally post op. Factor in for possible DVT (deep vein

thrombosis).

4. Don't get carried away with fancy imaging in terms of MR and CT

etc; decision to implant is clinical for the pain. Treat the patient

and not the picture. For OA, plain Xrays are usually sufficient to

establish the diagnosis. Longlength / measurement films of lower

limb are useful to establish pre and expected post op alignments.

5. Various types of implants are available, offering mildly different

outcomes; and unless you want her to participate in the Commonwealth

games, a simple and uncomplicated one is best. The type of implant

usually depends on the surgeon's familiarity and experience for her

situation (there are other considerations eg body weight, general

fitness level, morphological variations, her life expectancy, etc)and

so do not insist on a make which the orthopod is unfamiliar with. You

dont want your mother to be his experimental guinea pig.

6. Remember the surgical adage: bone is like a woman; once violated

never again chaste. And all implants will fail with time. One tends

to go for replacement with the same surgeon and so it is important

that the surgeon is friendly and with whom your mother can develop a

long term comfort factor and trust.

7. Again I stress that the implant should be for pain. Mobility

should not be the top of your list - as that will create unnecessary

expectations. A Zimmer frame is non invasive and cheaper than TKR for

mobility.

8. If pain is not debilitating, and the OA waxes and wanes; I would

prolong conservative management for as long as possible. Implant is

not a full stop, just a comma. Get a second opinion from another

orthopod and have a nice chat with your mother as to her problems.

For treatment to work, not only does the surgical bed need to be well

prepared, but the mind also needs to be receptive.

hi,

1.I am mani of 1994 batch. i did my Ortho PG and now working in chennai.

2.Further i agree with Muthu Sir that PAIN is the first concern and after aTKR

the patients Pain is gone!!!. Following this is the quest for better mobility.

hence mobility becomes a concern i do not say that u have to be in the olympics

but a good range of movement is provided by some implants like the HIGH FLEX

type of implants form S & N and Depuy. Zimmer has a new implant and yet to gain

poularity and recognisition

3. I have assisted some leading surgeons in Chennai and i know for sure that

Surgeon matters the most.

4. Regarding getting TKR done one knee after the other is time consuming i

know surgeons who prefer doing both the knees in a single go. advntgs: same

anaesthesia same surgeon and same recovery timeand cuts down cost by 30%.

5. I would be glad to help if u want the surgery to be done in Chennai.
