Guest guest Posted January 1, 2006 Report Share Posted January 1, 2006 Okay, I'm fine now (for the moment anyway) and able to think clearly enough about the events of the last two-and-a-half days to be very angry! I'm just wondering if anyone has any feedback about my doc's office because I'm VERY angry now that I'm putting the whole thing together in my head. Keep in mind that I like my doc very much, but she is very hard to get an appt with. I called a week or two ago wanting to get in because of a flare and they said my appt on Jan. 12th was the best they could do. I called several days after that with pain and they called-in some Darvocet. This past Thursday afternoon, my knee began to hurt and got to the point on Friday that I couldn't walk without assistance and I was in excruciating pain. I called the doc office and they were closed until Tuesday. The answering service gave me a very pat, matter-of-fact " we're closed until Tuesday " . When I pressed on, I got " the doctor on call doesn't call-in pain medications " . I didn't even ASK for pain medications! When I told the answering service that I was pregnant, it was like she was terrified to page the doctor. I wouldn't let up, saying that I wanted to speak with a doctor and she finally said she would have the on-call physician call me, but that she wasn't going to tell him that I was pregnant (presumably because he would be angry with her for bothering him with someone he can't prescribe drugs to, even though, according to her, he won't prescribe anything over the phone anyway). So, this doc obviously sees his role as to use a prescription pad and THAT'S ALL - that he has NO other service, wisdom, comfort, expertise to offer???!!! Anyway, he called and I explained my situation. He said I could probably take the Darvocet if I really needed it, but he asked me NO questions, gave me NO advice on whether or not to move my knee or use hot/cold therapy or increase prednisone or knee brace or ANYTHING. i find these things out from my online buddies. He said if I had to be seen, go the ER. He didn't mention that pregnant women can't get cortizone injections. I had no idea about this until my husband found this out from Walgreen's. Good thing I didn't spend a painful day at the ER only to be informed of that and CHARGED for it! I guess you have to let them hear you in your full dramatic, screeching agony before they " get it " . that's sad. i plan to tell my regular rheumy all about it, along with my frustration about not being able to get appts with her when I really need them. i like her so much, but i cannot con't to use a doc that i can't see when i'm most in need. i'm also going to talk to her about how i tend to be doing really well when i see her in the office and that i'm so conscious of drug-seeking (I work in psychiatry) that i'm downplaying my own pain, at times when i really should make quite a fuss, because i'm so worried they'll think i'm just drug-seeking. anyway, what do any of you think about my on-call experience?? When sit rheumatology they should discuss all the cases at " rounds " . I have no idea what kind of doc this was that she called for you...sounds like a quack. I would get in to see an OB doc and get a referral to a perinatologist if you can. You case is going to be complicated due to your RA and probably needs for meds throughout the pregnancy. Then the two docs need to confer on what is best for you. Most docs will NOT prescribe anything to a pregnanct women...they make the OB doc control all meds....its liability issue....so hussle on in to see the OB and get the ball rolling. Guest guest Posted January 1, 2006 Report Share Posted January 1, 2006 our ER just pages our normal doc anyway- and our ER tells us straight out they only take care of life threatening emergencies, and they have us sign a paper absolving them from any liability concerning dx, mostreatments etc based on idea they only treat a life threatening emergency anyway... BUT when I cannot get a doc to answer and if I think we need help NOW..and answering service won't page doc.then I do go to ER and tell ER hey my docs answering service would not page our doc. I am only a lay person and I sure do not know just what a life threatening emergency is but hey I am in pain.....and I am in pain NOW. I am sorry my pain does not hold any regard for my docs office hours or scheduling abilites. Truth is, I am a nurse. But people have the right to have pain treated NOW> (unless there is a reason they need the pain to reach a dx) Bummer is I had a nasty fast flareup of fingers on Labor Day..it came so hard and fast my rings got stuck on my fingers. I had to get them cut off. I went to ER and said I know I have RA and I am having a flare, and I need my rings cut off (my fingers were turning black already- within 2 hours and I had tried every known way to get them off already) ER insisted I HAD to see doc, I HAD to go thru all this red tape- had to have dx confirmed- oh but then since they are an ER they cannot anymore confirm a chronic dx...I was like hey look, I only need the rings cut off, I want no other treatment, I need no other treatment...I just need your ring cutter tool....everywhere else who might have a ring cutter tool is closed right now and the rings need to come off NOW or I WILL be having a medical emergency. I do NOT need labs and xrays etc. Grrr. Policy is policy....we have to follow rules. $1500 dollars in medical care later....and 6 HOURS later- my rings were a huge sad mess.....(they did not know how to use the ring cutter, broke my finger in the process and then mangled my rings so bad the jeweler could not repair them) I don't like our ER (have many other horror stories as well, and 3 friends who sued our ER and won...) BUT if my doc is unreachable- I will go to ER. People should not have to just sit it out if they have severe pain. Guest guest Posted January 1, 2006 Report Share Posted January 1, 2006 I know what you mean my rheumy is also by herself, but she usually has a nurse on duty and will call her at home if need be. I love my clinic I go to for everything else. Doctors and even emergency same day clinic all in same building so they always have your records. If you can't see your doctor they will send you to the same day clinic. If you take yourself to the same day clinic on a weekend or at night the notify your regular doctor about was was going. I hate the fact that they don;t have a rheumy Right now. He had a stroke,and is not pr acting right now. My late husband had RA and in all the years I was in nursing I never saw a person suffer as much as he did. One weekend he was in excruciating pain and I called ER. The male nurse I spoke to said they didn't take RA patience. I was never so angry. My husband was in pain all weekend. He died in 1994 from complications from RA. Guest guest Posted January 1, 2006 Report Share Posted January 1, 2006 , I am not surprised with what you've been through, after working at an afterhours answering service for years, and answering high-priority medical lines. Many docs don't want to be bothered afterhours, and leave strict instructions for answering service staff to tell patients to go to the ER. Nothing more frustrating for people answering the phone at an afterhours service to NOT be able to help people in need. If we dared to page or call the on-call doc if there was an emergency and the patient needed to speak with him/her, we usually got a nasty call from the doc or the office manager the following work day. I have gone to the ER here in Canada when the pain is out of control and I can no longer handle it with the meds I have. Keep in mind, the last time I went, I was wearing 2 of the 25mg fentonyl pain patches and taking up to 2 Percocets on top of that. Still, the pain was out of control. They did blood tests and my SED count was 54 (I think), and the high end of " normal " for a woman was in the 20's. Once that bloodwork came back, they couldn't give me a shot of Morphine fast enough. They were asking me what I wanted for the pain as the blood work backed up what I was saying. The doc and nurse even asked me how I managed to cope with it when it was that bad. I know what you mean about seeming like a person who is only seeking out pain meds. Prior to being diagnosed with RA a year ago, I would never even take so much as a Tylenol unless I absolutely needed it. I was the person who refused pain meds through IV or shots, and would take a Tylenol if I really had to have something. RA has brought me some of the worst pain I have ever experienced. I have had gallstones, natural childbirth that lasted 3 days of labour, and a tonsillectomy when I was 33 years old. All of that combined wouldn't even come close to an RA flare. I also have a wonderful Rheumy who is kind, compassionate and understanding. When I called her office at the end of November to let them know that I needed an appointment ASAP, as I had stopped all RA treatment a month earlier because of side effects - (which turned out to be MS symptoms undiagnosed), I was told the earliest I could get in was the end of January. Can you believe a 2 month wait with no RA meds??? I go in to see her in 4 weeks, and I will vent then. Until the end of the month comes, I will see my GP every 2 weeks, and he's been wonderful about helping me out with pain meds, prednisone, whatever I need to get through this. He has been available 24/7, and without him, I don't know what I would do. Sometimes, having a great GP makes up for it all. I don't believe my son realized the pain I am in until he got osteoarthritis. I rarely call him to help me because of my RA but when I do he comes as soon as he can.
