I use excel for two purposes – one is to generate labels

via mailmerge for mass mailouts as my EMR does not do this (is very painful). The

other is a pt database/registry. I have the following columns: first name,

last name, phone#, address, city, state, zip. Then the clinical columns: HTN

(yes ‘Y’or no ‘N’ and date of dx in mo/yr if month is

available), HTN controlled (Y/N), DM (Y/N and date of dx), DM c HgA1c <6.5 (enter

their HgA1c value and mo/yr), DM >6.5 (same info as prior column), foot exam

(mo/yr), UM (mo/yr c a ‘^’ if is high), eye exam (mo/yr), chol (just

put a ‘Y’, not putting a dx date in, no real reason why), and chol

controlled (Y/N and mo/yr of last lab).

For my metabolic syndrome/pre DM pts, I put their HgA1c in

the <6.5 column w date, but don’t have a ‘Y’ in the first

DM column, as they are not diabetic yet.

I don’t have dates for my HTN controlled column, as

only 1 of my pts is not to goal. Otherwise, I would have the dates of their

last BP log in there (have the BP log dates in the ‘alert screen’

on their facesheet in my EMR).

I don’t have the goals listed in excel, but is in the

chart. With a small pt panel (400), and my strict guidelines, its easy to see

who is out of line. E.g., for chol, all have LDL goal <70 unless they have

nothing else going on (no metabolic syndrome).

Hope that gives you some ideas. It is labor intensive, and

I have it running in the background while I’m seeing pts to update foot

exam e.g., and also while signing off labs. I’ve been trying to see if

DocSite would be better, and not more laborious.
