Guest guest Posted January 5, 2007 Report Share Posted January 5, 2007 There is an article (‘Improving office practice: working smarter, not harder’) in the Nov/Dec 2006 issue of Fam Pract Mgmt that has a pre-appt questionnaire that includes a ROS. I adapted it, making it less medical jargon and more lay person-ish. I’ve just implemented using it this week, and it has been helpful at uncovering symptoms I may have missed.
