Also, what exactly are you referring to in regards to “integrative, wholistic medicine?” That is more of a catch phrase or canned phrase & has a lot of different meanings to a lot of people. The trouble you will find in practice in regards to that stuff is that most insurance will not reimburse any of it, and unless you are in a fairly affluent area or can somehow achieve “guru” status, many people simply will not pay for it out of pocket. For example, I have considered adding acupuncture. However, the gold standard course (Helms Medical Institute/UCLA) costs around $5,000, insurance will not cover acupuncture, & people in my community would only want it if it is covered by “an office call” (ie, I accept whatever insurance pas for the day’s E/M visit (99213, etc) & do not charge the patient directly. I’m sure things are different some places but not here in rural Midwestern towns. chest xray OK, all you smart people out there. I am in Olympia, Washington, where there are lots of naturopaths and patients interested in complementary therapies. I plan on a cash practice with discounted rates for Medicare/Medicaid. I wouldn't consider myself a " guru " but have done some trainings to advance my knowledge of acpuncture and natural medicine. If so, why? Also, I’m sure you are aware that you can not charge cash patients less than you do the Medicare/Medicaid patients because that would be fraudulent. Everyone has to be charged the same for the same service except that you can give a “day of service” discount for those that same day. Yes, I’m sure medicine in Olympia, Washington is a world away from what it is here in central Ohio. As far as I know, there are essentially zero “naturopaths” here. I’m still not convinced though that I would be doing better financially by not taking insurances, hassles and all. Cash only sounds great on the surface but you might be surprised at how non-receptive people really are to paying out of pocket. I will give them discounts because i still want to have medicare/medicaid type patients in my practice and i'd like to transition those patients from my residency practice. Does cash work? you might be right. i may be getting my UPIN number and signing up for insurance 6 months after i start, but in order to have fun in medicine, enjoy my job, and avoid burnout, i'm going to take the dive and give it a shot. I have a couple families from out that way who come see me in Worthington, they are not your basic rural Ohioans, I'm seriously betting there is a cosmetic market in that area. . Still, I've never had anyone ever ask about Botox (or acupuncture or any other " alternative " treatments really). Also, is it really that big of a money maker or do you just really like offering it for other reasons? I'm not that into cosmetology-like stuff. I still find that very hard to believe that does not raise your malpractice. Injecting a poison/biological toxin into someone, one that could paralyze or worse (potentially, rare I realize)? My personal professional goal is to become an expert at OMT/musculoskeletal medicine as I think that is a great market/need AND insurances cover it, and you can code an E/M plus -25 and CPT code together.
