Insurance coverage for ENBREL may vary depending on the health

insurance company and the individual plan you have:

Private health insurance companies—Most private insurance plans,

such as the group health plans offered through an employer, cover

and reimburse for ENBREL. However, some managed care organizations

require prior authorization.

Medicare—Starting in 2006, Medicare may help cover the cost of

ENBREL. The Medicare Prescription Drug Improvement, and

Modernization Act will allow certain health plans to offer

prescription drug plans that may help pay for the cost of self-

injectable biologic medicines like Enbrel. Call 1-888-4ENBREL to see

if you may be eligible for Medicare prescription drug coverage. To

learn about recent changes to Medicare, please visit

www.medicare.gov.

Medicaid—Currently, all state Medicaid programs cover ENBREL.

If you're unclear whether your health insurance company covers

ENBREL, call 1-888-4ENBREL, (1-888-436-2735), and have our insurance

specialists find out for you.
