Has anyone ever written a letter of appeal to get their child treatment for CBT

and ERP? My insurance will not cover the treatment so I am trying to get

in-network benefits with out-of network doctors. Not one of the doctors in my

network does the CBT and ERP. I need to write an appeal letter to see if I can

go to an outside network doctor and receive in-network benefits. I was wondering

if anyone had a sample or ideas of what is needed? Thanks so much!
