I don't have any answers for you, Dina, but am sending good wishes your way.

Hope you figure something out soon. It is a pain having to wait so long to see

what works and not knowing whether a change will make something better or worse.

Is it possible to increase her Prozac dose? Is that something you have tried

before?

I'm sorry your daughter is having a hard time again. My kids have done Zyprexa,

Geodon, Risperdal, and Abilify. The Risperdal made my younger daughter's

cholesterol high and the Geodon didn't really work for either of them. Zyprexa

was one she was on when she was really young and we stopped giving it to her

because she was doing well. We switched doctors (ours moved) and the new one

put her on something different. The Abilify works well and is suppose to be low

on the weight gain scale but both of my kids gained weight on it. My younger

one dropped about thirty pounds when she stopped taking it and my older one was

off of it for a few months and dropped about twenty pounds. I also noticed that

the older one was a lot less sleepy when she was off of it. She has narcolepsy

so the sleepiness might not be a normal side effect. The older one did not do

well off of the Abilify so we put her back on it about a year ago. I'm kind of

rambling so I hope I'm making sense. All kids are different and have different

reactions. Good luck. Kelley in NV

Augmentation with Antipsychotic

Hello all! Well, all good things must come to an end. My 14 yr. old has been

having a bad time with her vomit thoughts since the beginning of the school

year. I added back her Buspar, which has worked twice in the past few years, but

no dice. It's not even touching her anxiety. She is still on 60 mgs. Prozac.

Also in the past, we've augmented the Prozac with an antipsychotic. Zyprexa was

the one we've used 3 times, but since she isn't having trouble eating I don't

want to use that one since it's such a weight gainer and she's a dancer. We have

also tried Risperdal briefly, but that only increased her anxiety. I am calling

her pdoc in the morning, but would like to have a plan for what I want before I

go in. I was thinking of asking for Abilify. Can someone tell me side effects

they've seen? Weight gain? Zyprexa made her congnitively dull. Do you see that

on Abilify? What doses?

We could also augment with another SSRI, or Luvox. I hate to take her totally

off the Prozac since she's been on it for 7 years, but it may be time for a

change. I really with you could tell more quickly with these meds, it's so trial

and error. I am wondering if the Prozac is helping her more than I think it is?

There have been a few kids on this site that have gone off to try something else

and if they went back on it wasn't effective. I'm so scared of disrupting her

life more than it already is. She's got a nice group of friends and dances on a

team 4 nights a week. There's not a lot of room for trial and error.

Thanks for your input. I hate this disease!!

Sorry I can't very much help with medicines? I have the 11-yr old with similar

vomit phobia and she is on 150 mg Zoloft. She was doing well for a while, but

lately not so good. The dr. wants to put her on a second 'booster' medicine,

Risperadal, but she really fights medications...she won't swallow pills since

she's afraid she'll gag on them and vomit! So I'm not sure what we will do. My

daughter refused to go to her dance lesson today since she wouldn't ride in my

" contaminated " car (where her little brother threw up 3 months ago). She went to

a junior high dance Friday night and hid in the bathroom to avoid a " slow "

dance. While she was in the bathroom hiding another little girl came in sick and

threw up in the next stall. My daughter had a full panic attack and called us

crying to come pick her up. Her shirt, jeans, and favorite sweatshirt from that

night are now " contaminated " too, and she won't wear them even though she

herself wasn't sick, and even though I've washed everything in hot water! There

is no reasoning with OCD. I hope you find the right medication combination for

your daughter, and sad to hear how tough it is. These kids really suffer and as

moms we do, too! I just feel so powerless to help her. Joanne

Dina,

My son hasn't taken medication so I know absolutely zero about it, but my nephew

is on quite a bit and I have learned lots from reading and listening.  I

thought there was some new medication that people were having lots of success

with that is actually for seizures -- is it trileptal or something like that?

I have only heard good about it augmenting an ssri.

I hate to hear that about your daughter.  I hope things get better quickly.

Joanne,

Sorry to hear you are having such a tough time. Is she doing any ERP? An ERP

therapist can also work on pill swallowing. My 9 year old takes liquid Zoloft,

but I have given her Ativan once or twice in applesauce. It is a very small

solid pill. I think I remember from this summer that your daughter wasn't eating

much? Is she doing better with that?

Sounds like the sick girl at the dance may have contributed to the anxiety

spike. Sometimes when I cannot reason with my daughter and she swears it is not

OCD, but just that she is really " germy " this time, I will ask her if I talked

to her friend X would that girl say this is a good idea. If she says no, then

at least at some level, she still knows that this is not rational behavior.

(So, for instance, If I ask X, do you need to wash for 20, 30, fill in the blank

minutes after using the toilet, will she say " yes, I do " . I would, of course,

never actually ask the friend this, but it seems to help at least a little bit

to see that this is OCD talking, not my daughter.)

Hope things get better soon.

Hi Dina,

My daughter is on abilify (risperdol worked well for years too)

and it " saved her " from our having to consider a therapeutic school.

(She never had a problem in school but the anxiety was so high that

she exploded when she came home). So, I am grateful that abilify has

worked as well as it has. However, I do believe it has contributed to

her weight gain of 20+ pounds in a year. For my DD the abilify was

prescribed because of aggression and rages. Her OCD doesn't seem to

be " touched " by the abilify. She is on lexapro for the OCD. In fact,

OCD is more of the daily problems now that her aggression is under

control (her psych believes the control of aggression is due to the

abilify and trileptal)

I know it is very difficult to get the doses right when you

combine an antipsychotic and an antidepressant. I've never heard of

that being done if OCD is the only diagnosis. So, I was surprised

when you termed this subject " augmentation " . Best of luck with this

difficult time.

Hi Dina,

I'm sorry your daughter is having a rough time!!! It's such a roller coaster

ride, isn't it???

As you know each med is different for each kid. Both of mine gained wieght on

Abilify , even though it is suppossed to be " weight neutral " It also affected my

dd cognitively, but not my son. My kids were both on a starting does of 2.5mg ,

than worked up to 7.5mg

Personally, I would leave the Prozac alone, and either add to it ,ith another

SSRI ,or an antipsychotic, if her ocd is worsening.( my opinion only)

Let us know how she makes out!!

Hugs

Judy

Hi Dina, just my thought that I'd stick with the Prozac and try augmenting it.

She's been doing well on it so I would hesitate to take her off/switch before

trying other routes. As you said, it may be helping more than you realize. I

know of other adults (from reading in their group posts) who have stayed on the

same med, including Prozac, always for their OCD but doesn't mean they still

don't fight/feel OCD at times; but OCD improved well on the med, helped a lot,

etc. -- Yeah I agree, hate it's so " trial and error " and takes so long to

" trial " etc.

Again, just my thoughts.

So sorry. We are kind of in the same boat here. It sucks. It's hard to have

them stable for a long while, then everything starts again. I guess that's the

nature of it, but it's still hard to see it come back and be at work in your

child.

How long has she been this way? Is it something that might pass if she is able

to push through it? Josh's anxiety has calmed down a bit, but I see his OCD at

work. He is fighting it, with my encouragement, implementing the ERP he

learned. I'm hoping it will just calm back down at some point, altogether. I

hope and pray the same for you.

I feel the same as you. . There is never a good time to change meds, it seems.

The trial and error period disrupts their lives.

I hate this wretched disease too.

Thank you...Yes, there is a liquid form. Sadly, she won't take that due to her

fear that it has a bad aftertaste. The dr. had made her pick how she wanted to

take the medicine and she picked the pills. She doesn't actually swallow the

pills though...she crushes the pills and then mixes the powder into a spoonful

of chocolate pudding every night. Some nights it goes smoothly (like tonight)

but other nights we get tears/tantrums over having to take the medicine. Joanne
