My son had dental work doen under iv anesthesia from the age of 15-20. Usually he was ouot of my sight for about an hour. I stayed with him in the OR room until he was out. I was with him as soon as the work was finished until he awoke. When he woke, sat up and talked to me, we went home - no recovery, no invasive anything. I cannot help with most of your questions, but I do know most doc's prefer to put our kids under general anesthisia God forbid something happens during the procedure. This helps keep their vitals stable. Tori has been put under General for an MRI that was 1.5 hours long. She will then be getting general for arthroscopic surgery she is having done on 4/1. Guest guest Posted March 11, 2011 Report Share Posted March 11, 2011 Both IV sedation and anesthesia have risks, but it sounds like the dentist and anesthesiologist are giving you the " your just the parent what could you know " treatment. That just gets me so ticked! You are the Parent and you ARE entitled to know EVERYTHING. Aden had caps at 7 and they did them with IV Demerol. You might ask your dentist why that isn't an option for Luca. They may have a reason, and they should be willing to give you that reason up front. When Aden had his caps it did take a lot longer for " recovery " because they had to monitor his oxygen level until most of the meds had worn off and then he was really " drunk " for several hours afterward. They sedated him before 8:00 am and we couldn't go home until around 3:00 pm. The actual caps took about half an hour. The rest was recovery time. In my experience the dentist can usually tell you what they plan to prescribe after surgery (in fact they usually have us pick it up before hand so we have it on hand when we go home without having to stop at the pharmacy) we've had a couple of times when they added or changed something but not a lot. If it was me I wouldn't do anything until they answered all my questions with answers that made sense. I take the view with non-emergency medicine or dental that if I don't understand we don't proceed until I do. Bottom line, don't do anything you don't feel comfortable about. He's your kid and you have every right to be informed about his care! Just a side thought, you might call your insurance and explain about the anesthesiologist not accepting insurance and see if they have anybody contracted that works dental. Guest guest Posted March 11, 2011 Report Share Posted March 11, 2011 We had a problem finding any dentists that would sedate even for routine dentistry simce would not even sit in the dentist chair and since he is about 170 pounds my wife and I could no longer put him in the chair. Not that it would have mattered. We found a clinic called Special Smiles in Phila PA run by Temple Univ Hospital. They sedate in the waiting room and do xrays and then talk with the parents and do all procedures needed. The visit took about 2-3 hrs and recovery was fairly smooth. They gave both boys shots to prevent that sick feeling from the sedation. They require a full physical exam before they will make the second appointment and the first appointment you meet the dentist and he explains the program. Our insurance covered every thing and the working was done at the Temple U Hospital. This was the only place that we could find after several calls to local places that said they do sedation dentistry. All did procedures under sedation but not rountine checks. If you are not in this area I would check around there should be some place you can turn to.. Thank you, Great. that's what I wanted to know. That's what I'm thinking go slower so Luca can be eased through. I don't like this idea of going under for 3 hrs and doing so much work at one time. I'm going to back this up or go to a different dentist. I hate the fact that they want their money now, when the dentist hasn't returns my calls with questions of why he has to do this one option. I get the office staff when I call they put me on hold then go ask the dentist my questions. I get the same answer he does 3 hrs of anesth with all disabled adults. I was told he won't do IV sedation on the disabled adults. If he can't address my questions directly with me ahead of time, then I'll take my son elsewhere. I was not going to decide what to do until we met with his new primary care on Tues. But I don't care for the business practice that says pay now, I'll explain later mentality. And as I said I wanted him to order meds ahead of time Lu is scheduled to go under at 3 pm. The pharmacies close at 7. The dentist office is 1/2 hr from home and our pharmacy if they run late? We won't be prepared. Even when I explained that to his staff they put me on hold, supposedly talked to him, came back to the phone and said he will call you today. That was a week ago. This is plain and simple common sense! I did call my insurance several times. I do have 2 more dentists I'm planning on calling. Also was told more anesthesiologists in our area are not accepting insurance and asking for prepayment, but not so soon in advance. His fee is average for our area too. I would get a statement the day of surgery showing I paid, then I can send that to my insurance and get a partial reimbursement. My husband is no help he doesn't have the neurons in his brain to think things through and be of practical help, he became disabled himself 6 yrs ago and had to retire at 49 yrs old. I'm tired from care taking 24/7 in my own home...and needed input from the outside to help me think about this. Yes I am interviewing others to help me in our home. The problem is...my husband! He's a bear to work with, Luca with all his self stim behaviorial issues is easier. Although Luca is a past patient there, right now there is a 6 month waiting list to get in! It's frustrating & hard to go outside that comfort zone of the hospital for his dental needs. My son has had only one procedure where they used anything BUT IV sedation - that was JUST IN CASE when he had his T & A at age 8 - and even then, the entubation was to prevent swelling of the trachea. I have never heard of ANY of the adults who used the clinic we did being under for three hours - even with multiple fillings - and extractions take no time. Elie had x-ray, cleaning, fillings and sealants in one visit in under an hour with iv sedation - and EVERYTHING was given to me in written form as to how they would do everything. My 20 year-old son had his first dental surgery a few months ago, but he still qualified to have work done at our local Children's Hospital. Our pediatric dentist was adamant that we get it done there and not wait until he was older because the anesthesiologists there are used to taking extreme care with any special considerations required. He was under for about 2 1/2 hours, and had two wisdom teeth extracted, four teeth filled, and some really good teeth-cleaning done. This was only the second time he has been under general anesthesia, and I was pretty worried about afterward. Everything went extremely well, but I also didn't have another dependent adult to take care of and my son will take meds orally. I know this is no help to your current predicament, but I will tell you that in our area (central Ohio) we have some dentists who practice " sedation dentistry " or " painless dentistry " , with a variety of sedative options offered. I wonder if you have someone in your area like this? The thing is, they do this ALL the time, have considered all the concerns that patients have, and have dealt with people who have avoided going to the dentist their whole lives because of fear. You might have already considered this idea, or the cost might still be too prohibitive, but just one more thing to throw out there. Sounds like you are doing all the right things, to keep hammering away at this until you find the solution that fits your family's needs. Good luck! Keep us posted. Is the dentist willing to let you be by your child's side while the child is asleep? If not, then no way would my child be put to sleep for even 5 minutes. We stopped going to one dentist because they a) would not let the parent back with the child (we never followed through on the first appointment after arriving), and they wanted to give my child narcotics without my presence. Those are two things that just ain't happening without my presence in a dentist's office. Shortly after my refusal for the dentist to do anything like this, there was a clip on the news with that same dental company in GA accused of wrongdoing while putting kids to sleep. We have had our share of difficulties getting dental work done under anesthesia. At age 5 Zeb had a small filling done in the dental office. They used a small amount of Novocaine and he did fine. I was driving home on the interstate and he starting biting his tongue, didn't like the feeling of being numb. Blood was squirting everywhere and I drove as fast as I could to the nearest exit, precellphone days. I ran into a convenience store/gas station grabbed a popsicle to get into his mouth. I never paid for it and just said call 911. He bite his tongue almost in half. It took a month to heal and was huge and swollen. He barely could eat. it was terrible. Since that time we have had all dental work other than cleanings done under sedation in the children's hospital. Our regular dentist retired and another dentist bought the practice. We stayed with the practice because Zeb was familiar with the hygienist to clean his teeth. He has has no major problems since he was young. He recently started complaining about a toothache so this new dentist just wanted to pull it out in her office. I told her no and why.She just didn't understand the situation. She told me that we needed to see an orthodontist. I took Zeb to an orthodontist that told me that he wouldn't touch the kid without having his underbite repaired, a major surgery. He has never had any trouble eating and I was never told he had an issue with an underbite. I was advised to take him to our university dental school. He saw an orthodontist there that told me he did have an underbite but would not adive it to be repaired unless it created a medical issue, TMJ or eating difficulty.He did not need braces and the teeth would shift to fill in that small space after that tooth was removed. We moved down the hall to see a regular dentist that insisted she could just pull the tooth in the office. She tried but Zeb was not cooperative. They would not put him under because it is a very small tooth. We are home and Zeb still has a toothache. It has been two months since we saw the first dentist and we are no better off. It's so frustrating. My niece is a dental student at UCLA. If anyone is near the LA area I recommend using that dental school. Have you thought about TUFTS - is that near enough to be an option???? i hear that they are awesosme. 2 of my kids have used their dental clinic when they were in school near there. Couldn't one of these dental geniuses have used an IV anesthesia that is quick in and quick out????? Or oral VERSED??? We used oral versed to pull one of Elie's baby teeth. We have NEVER used nococaine on him. He had many little fillings as a young child by the hang on to the kid and work fast method!!! Guest guest Posted March 18, 2011 Report Share Posted March 18, 2011 Don't know if I respond to you or not...I'm coping but a wreck. I'm meeting with the RN nurse this afternoon for my piece of mind. Luca's dental surgery will be backed down to tolerable/reasonable procedure, we tried the IV sedation; he ripped it out. That was AFTER Valium, and he didn't go to bed til 3 AM! (I would have been in a comma) Not Lu! Therefore anesthesia it is. Dentist is the sweetest most loving woman we could ask for! I " m pleased as hell!!!! She is all about his comfort and gentle care! We met the new primary care. Lu almost didn't go to the appt! It was hell getting him there. He needed his hair cut and some grooming done and LORD he dressed himself! What a hoot, nothing matched! But he had his favorite Bulldog shirt on and he was leaving; I let it go. I really liked the Dr and her assist, Lu liked both of them too, Lu was cooperative and kind.The Dr was careful about his first appt being positive so he would return. She noticed immed that he is on the Spectrum and said we would discuss that after he heals from his dental work. I was impressed she noticed. I came home so relieved...Lu actually has a primary care again! Then the problem came with the meds. We thoroughly discussed them at Lu's appt. I KNOW what the Dr prescribed: the Fentanyl patch, an anti fungal cream, (fungus by his ears) poor guy, eryth 2% gel for his back rash, and oral amox antibiotic. Crossed fingers he would take the antibiotic! First does yes, next dose flew across the room spraying a rasp/straw sticky mess from the floor to ceiling splattering on me too. (another ruined shirt) Reason for the patch is he doesn't do oral meds. He needed something for pain now. The last med that took a couple of days was the anti fungal cream. The pharmacist calls and says she can't read the prescription and wants the Dr's office to clarify, the Dr is gone til Mon, nothing to do but wait. Am hour later I get a call from the pharm the prescription is ready...hmm...weird but cool. When I pick the last needed med it's in tablet form. I told the pharmacist this is wrong no tablets, this is s'pose to be a cream. She said the Dr wrote tablets. I left a bit confused whose to blame for this problem I KNOW the Dr said cream! The pharmacist had no warnings to share or instructions. I got home and read all instructions as I always do...thankfully I did. There is a drug reaction from mixing 2 of his meds; it's very clear. Fentanyl patch and Ketoconazole tablets do NOT mix. SO, the pharmacist we've used for years missed this. And what? Does this mean the Dr we were so pleased with made a mistake and wrote TABS instead of cream? I know she said cream!! The only oral med was the amox. I will go back to the pharmacy and ask for a copy of the orig prescription and get to the bottom of it. I'm not threatening lawsuit but I will and must confront and make others aware of mistakes on Lu's behalf! This could have been life threatening for him. Anyone else having these types of issues? WHY is life so HARD sometimes. Pray for me, I need life to lighten up a bit! My son was a terrible medicine taker before he learned to swallow pills. I remember putting it in pudding, applesauce, yogurt, ice cream, hot fudge and whatever else I could think of. I hope you can find something that he will like.
