please advice for monthly report making.

I am planning to make a report for monthly safety statistics from the

projects. So what shall be the items included in the report. For the

time being I am planning to include total man hour, safe man hour,

safety highlights, near miss overview etc. what shall be the graphs

and new items which can be included, please advice.
