Guest guest Posted August 7, 2010 Report Share Posted August 7, 2010 I think the Medela shields have a slightly more flexible silicone. I use both 24 and 20 mm shields. It really depends on the fit for the infant, and the mother's nipple size/elasticity. It's not always an exact science unfortunately, it also depends at what junction you are working with the mother, meaning the elasticity of her tissue and thus the fit immediately after birth is sometimes different as opposed to after lact II. Sometimes mothers with more generously sized nipples can have friction and so forth when their tissue is elastic and this resolves after their breasts become more full. I always talk to mothers about this and give them information about what to look for and sometimes we try 2 sizes and evaluate for a period of time which is most suited for both baby and mother. I have worked with LGA newborns who achieve best results on a 20mm and SGA infants who achieve best results on a 24mm. H.
