We never lived anywhere long enough to even warrant applying as the waiting lists were so long! Since we are now here in Maine for the duration, I am applying. Medicaid is called MaineCare here. I know we don't meet the income requirements for that so I am applying for the Beckett option. I know other states have Beckett so maybe you can help. I just rec'd the forms in the mail today. I have many questions that I'm hoping you all can help me with. Some may seem very obvious but I swear I'm not sure what to say - I don't want to do anything to screw up services that I believe deserves! 1) I have to fill out the MaineCare regular application - do I fill it out as since I know we don't qualify, and I? I'm applying for HIM for Beckett so do I fill out all the paperwork as HIM? There is a separate application for the Disability side of things... 2) Do I list ( my husband) as a person living here under the " PERSONS WHO LIVE WITH YOU " section even though he's not technically living here right now? He will come May but only part time as he'll be stationed in Mass. So confusing - it's not like we are divorced or separated - we're just living apart due to his job. 3) DISABILITY INFORMATION - " Date your illness or injury first happened? " I put, " Birth. " " Date finally disabled? " What do I put here? " Please describe your illness or injury; " I put " Down syndrome and diagnosed with Autism in 2004 " " How does your illness or injury limit your ability to work, to be a homemaker or limit you in other ways? " PLEEEEAAASSEE - where do I start? Any ideas? " What advice has your doctor given you about your illness or injury? " UM???? In the next section - MEDICAL SOURCES - " please list all doctors, clinics and hospitals you have visited. Doctors and mental health providers (psychiatrist, psychologist, counselor, mental health agency) " 4) Do I list every doctor in his life or just the ones we've seen in Maine or just the ones over the past " x " number of years? Same goes for Hospitals... 5) It asks about OT, etc... Do I say yes, since he receives these services through school. Sorry to sound like such a doofus but I've never filled out this type of paperwork and with hubby gone I can't ask him.... HELP! Thanks guys, Jayne Ps. I left a message for the caseworker assigned to us with DHHS - just want to get a heads up on how to handle things - don't want to jeapordize 's ability to receive everything he could possibly qualify for!!!!
