After a few years of some success with pancreatin (Viokase) for Madelung's Disease (the non HIV form of what seems to be the same lipodystrophy that occurs with PI's) a suggestion that I might be gluten intolerant came up. Though I had not tested positive for any of the three antibodies, I seemed to have the lions share of the symptoms including recently developed hypothyroidism. I've been on the gluten free diet three months now and what a difference! I think it's worth asking to be checked if you seem to have the symptoms. It's just a blood test. Also if you already know that you are gluten intolerant could you please post something or if you find out that you are or go on the diet because you have symptoms could you please post something as this would represent yet another point of connection between HIV Lipo and other lipodystrophies.
