baby fussy with feeds

Got a phone call for a consult but with mom's travels I can't see her until next

week. ANY INPUT WOULD BE APPRECIATED!

HX: healthy baby boy, gaining weight well, approaching 6weeks of age, first time

mother; since 1week old baby has been fussy at breast and frequently producing

green (sometimes mucousy stools);; @ 2weeks of age had in office visit with Peds

LC to deal with above and fact that baby eats every 1.5 hours; her dx was that

baby was fussy due to forceful milk ejection and plan was to block feed; mom saw

improvement in temperament for about 1week but no change in frequency of feeds

or green stools.

Her call was followed by this email: my son's intolerance for the slow flow of

milk (the times of no let downs) have seem to have worsened! He even started

squirming during out middle of the night feedings and the first feeding in the

morning, to the point where he ended up in a major crying fit. He is perfectly

content during the letdown periods but gets very fussy and upset in between. He

would nurse for 5-10 minutes during the first let down and after that it is a

struggle to keep him on for more than 2 minutes at a time. I have been trying

switching breasts back and forth every time he gets upset and he ends up getting

about 5-6 letdowns per feed but it takes about an entire hour which is very

exhausting. I know I should have him on side A for 20-25 min and side B for a

minumum of 10 min but it is increasingly getting difficult to get him to stay

that long without multiple fits.

I guess some feeds are better than others but I am feeling very discouraged

since this is happening at the times when my supply are supposed to be the most.

I can tell I still have milk in my breasts when he gets upset and it really is

him just not wanting to work. When nursing is going well, it is such a

pleasurable experience but I am at the point where I feel I should give up and

exclusively pump since I am going back to work in 5 weeks anyway. HELP!
