I went to the Doctor today and they told me my 24 hour Protein was only 300mg. I often notice pretty heavy foam in my urine so I am confused as to how it is this low? I guess you can't judge it that way. Is there anything else that might cause Foamy Urine? Anyway my creatine was 1.9 and my creatine clearance 55. My blood pressure was high at 160/95. So the Doc. Guest guest Posted October 9, 2001 Report Share Posted October 9, 2001 You can't really tell how much proteinuria there is by the presence of foam in the urine. Even mild proteinuria will produce foam. In terms of other disorders, I believe there are some liver conditions that can cause foamy urine as well, but I really don't know much about that. 300 mg/d of proteinuria is pretty mild in the grand scheme of things. Most of the evidence that lowering proteinuria with any of the 3 classes of BP meds that have that effect seems to indicate that there isn't much benefit anyway if the proteinuria is mild like that. The main thing, in my opinion, is to get the blood pressure quite a bit lower than that, aiming for 120/80 or thereabouts - that will probably protect your kidneys in the long run more than anything else you can do. It's great if you can tolerate clonidine, because most people don't tolerate it very well. I'm sure your doctor has his reasons, but I find it puzzling that clonidine would be used before ACE inhibitors, beta blockers and angiotensin II receptor blockers have been tried in a patient with mild IgAN. You seem to be about where I was in the late 80's and early 90's - mild IgAN but BP that is relatively high for the amount of kidney impairment. In my case, high blood pressure runs in the family, on both sides, so it just made the blood pressure all that much harder to control. They used Clonidine as an emergency drug to lower it while I was in the hospital. Since I seemed to handle it fairly well they just left me on it. My blood pressure is still very difficult to control. When I was in the hospital my 24 hour urine showed I was spilling about 3.4 grams of protein. That has since gone all the way down to 300mg since I have been put on the tiazac. It just baffels me because I was once on an ACE Inhibitor for about 2 months and the foamy urine was gone completely. Now I have lots of foam sometimes, but the Doc says only 300mg per 24hours. I take the angiotensin II receptor blocker Avapro (irbesartan). Sometimes, I have absolutely no foam whatsoever, sometimes quite a bit, at different times of the day. And there is no pattern to it at all. I remember it was the same when I was on various ACE inhibitors, and also Cardizem CD. With BP meds, most doctors seem to operate on the principle of " if it ain't broke, don't fix it " . They would rather increase the dosage or add something else rather than have someone change meds. There also seems to be a 's Law in effect.
