Guest guest Posted July 11, 2008 Report Share Posted July 11, 2008 Hi! I am so very sorry to hear about your mom's recurring UTIs. My mom has had them - nearly without a break - for over 2 years. They are hard to combat, require constant monitoring, and anti-Bs need to be changed as the bacteria changes. If I'm reading correctly, you're talking about nitrofurantoin, the generic version of Macrobid and Macrodantin. I certainly can't speak to what would be best for your mom, but I'll mention a few things I've learned from the pharmacologist where I work (senior care organization with hospital) about this problem and meds. I think there's also a link to a long post from me about anti-Bs and the UTI issue - I won't repeat everything here. Has the urologist determined why your mom is susceptible to UTIs? It's important to rule out certain causes. Some of them can be addressed. You were smart to say no to Detrol. Does the MD think the UTIs are related to your mom's bladder not emptying completely? Of course Detrol isn't an anti-B. According to the PharmD, nitro doesn't have lots of red flags related to LBD as some ant-Bs do, but it will not work for people with low creatinine clearance levels. (My mom is one of those.) The CrCl is checked via blood work. I could give my mom nitro for a year and it would do no good. Many of the MDs here who work with seniors order a CrCl level test when they order the urinalysis/culture & sensitivity. Then they know what anti-B to use. Bacteria mutate. The cause of this week's UTI may be different from the one that occurs a month from now, so both the check for bacteria and the test to determine what anti-B to use should be done each time a UTI is suspected. Different anti-Bs treat different bacteria - there's no one pill for all bacteria. Many MD's don't order a long-enough course of anti-Bs. People with recurring/relapsing infections may need a much longer course of treatment. Certain meds can interact with certain anti-Bs. Readjustment of meds, dosages and dosing times may be needed. While a given anti-B may be " safe " in LBD, it may still affect the person. My mom gets very sleepy on certain anti-Bs, irritable on others. Take care. Her MD was at a loss as to why she continues to get these UTIs. The Urologist says her bladder has dropped, so it's prone to bad bacteria and prescribed an estrogen cream. He also told me that older people tend to not want to empty their bladder and this might be part of the problem too. And as a preventive measure order the macrobid. He has allowed for 11 re-fills with each lasting one month. Now I am wondering due to what you've stated, that by Dec. the bacteria might mutate and this antibiotic may no longer be of use. I guess if he sees her on a regular basis, he'll monitor her?? She goes back to the urologist in one month. Guest guest Posted July 11, 2008 Report Share Posted July 11, 2008 i personally take the generic for macrobid/macrodanton 100mg nightly as i have recurrent kidney infections due to a urostomy *i have no bladder i have a urine bag similar to what most people are familair with colostomy for poo urostomy is for pee) there is another prevenetative that Univ of Florida and Shands is Trimethoprim, it is used to prevent kidney and uti infections in people who get them chronically. i found that macrobid worked better for me than the trimethoprim but i did try it. so that might be another to check into. also some of the medicines like levaquni come in higher mg which might hlep eliminate the infections. it is best for them to run a urine sensitivity and culture with the standard Urinalysis *UA* that tells them which medicines would be better for that particular infection. good luck and hugs sharon From: ouryellowrose <ouryellowrose@ yahoo.com> Subject: Nitrofur Monohy CP 100 MG ??? One day I asked the nurse if it was staph, and she said there was some staph there. If that is the case, I don't think they ever get rid of it. Just suppresses it for awhile, and then as soon as the AB is done with, it comes back. He, too, was on Macrodantin to try to prevent it, but it didn't do the trick. I suspect it's probably caused by several things, such as you listed, but if there is staph present, I don't think it gets completely cleared up. Just my opinion. Guest guest Posted July 13, 2008 Report Share Posted July 13, 2008 Hi again - If your husband's creatinine clearance level is high enough, it's fine to use the Macrobid as a prophylactic. We use Bactrim for the same purpose for my mom. But since not all infections are caused by the same bacteria, one antibiotic won''t protect against them all. So watching for new infections is important. Urinalyses aren't typically done when there are anti-Bs in the system: the med has to wash out of the bloodstream to get an accurate reading. And yes, while many people are on preventive anti-Bs, there's also a chance that over time the body develops an immunity to that particular anti-B and it may prove useless in future. Bacteria get " smart " and adapt to their environment. And June's description of her husband's situation is a good reminder to insist on a complete C & S - culture and sensitivity - of what the bacteria is/are - and what anti-Bs are needed. Doctors sometimes don't want to treat what they consider a " low level " infection - or want to treat only the bacteria that appear in larger numbers, not the " lesser " infection. Multiple infections are not uncommon in older or compromised folks - the clinical literature is full of research on this. Best wishes to you and your LO. Please keep us posted.
