I was wondering how to type that. The signature line is flush right. Should there be a line space between the physician's name and title/address like this: _______________ , MD Associate Program Director Internal Medicine Residency 1234 Main Street Anytown, USA Or should there not be an extra line spaced between his name and title/address? Also, I have never seen an address at the bottom underneath the signature line since it is usually on the letterhead already. Hope this helps.
