Those with more neurobiological conditions and phenotypical traits of DS in their families are more at risk than those with lesser combined neurobilogical conditions and DS traits. I can honestly say that I am not bothered by an increased risk factor. If the risk factor was 100%, this would not prevent me from wanting or having another baby. Now, I am just smarter and I would do some things in a bit of a different order.
