Guest guest Posted January 1, 2001 Report Share Posted January 1, 2001 I'm curious if what I have is flank pain (kidneys) or something else. I have IGAN, Henloch Schlein Purpura. I've know about it for 3 years now. My year 2000 lab results range - creatine 1.1-1.4 mg/gl creatine urine 149-160 mg/dl creatine clearence 111-141 ml/min 24hour urine volume is 1000-2700 ml blood pressure 115/70 I'm 6,2 200 pounds Medications- 5mg Lisinopril per day (ACE inhibitor) Last year I asked my Dr why I have an isolated pain on my left side below my ribs and just above my left hip. She ordered an ultra sound of my kidneys. She also lightly hit my kidneys to see if they were unusually sensitive. Both results were negative. Still the pain contines and it is more noticable when I sleep on my left side. Sometimes I wake up in the middle of the night due to the pain. Sometimes it is a sharp pain when I bend over. And when I ride my mt bike and hit a large bump in the road or trail I sometimes feel a sharp pain. How can I determine if the pain I now have below my ribs and to the left of spine is flank pain and not something else ? Is the pain typically more often on one side of the spine or both ? Whenever someone says that the pain is on a particular side, I have to wonder whether it's the side the biopsy was done on (usually the left), and whether it might not be residual pain from where the needle tube went through the thick muscle tissue in the flank (and it doesn't have to be that recent). Even the best nephrologist in the world can't really say what the source of flank pain is in IgAN patients, because theoretically, you can't feel glomeruli. It probably has something to do with some localised swelling of tissues due to mild edema, and it may be the same pain as the stomach pain that is associated with HSP, except that it isn't in the stomach. Most commonly, it is a dull ache type of pain, but other members of this very group have described very different types of flank pain they experience. We all seem to be different. What you describe as the location certainly is what I refer to as flank pain. It's really hard to tell with pains anywhere in the torso, because there are so many nerves in there all over the place that are all interconnected, and it's so easy for pain felt in one spot to actually be referred pain from somewhere else. If you have it, it seems like this is just something that's on the list of things we just have to put up with. However mine says that he sees many patients with IGAN who complain of it, myself included. It was explained to me that the kidneys due to the condition, become oversensitive, and if you are feeling slightly unwell, whether it be a virus or otherwise, can irritate the kidney....but none of this has be proven to be medically so. Not exactly a mind blowing explanation, but they really seem to have not other information for us. Perhaps someone else from the group can explain this better for you. The biopsy was done March 1999 and the current pain feels exactly like the soreness I had after the March 1999 biopsy procedure. The Drs never told me that the biopsy might cause long term residual pain. Doctors are very non-committal when it comes to pain that they can't explain with certainty. Sometimes, even just blood pressure medications can cause some pain. This is why I always say that one shouldn't start more than one medication or treatment at the same time if it can be avoided.
