IMPRESSION:

1. Gastrointestinal bleed, probably last two months, etiology undetermined.

2. Anemia secondary to gastrointestinal bleed.

3. Hypertension, essential.

4. Diabetes mellitus, type II.

PLAN:

1. Admit the patient from observation to regular admit for gastrointestinal

bleed.

2. Treat the patient as upper gastrointestinal bleed. Will continue

intravenous Zantac.

3. Possible repeat endoscopy.

4. Continue treatment of diabetes mellitus and hypertension will current

medication.

5. Daily hemoglobin and hematocrit to check for the status of the bleed.

6. s/l dip-in-crosmutz ?tip-in-crosmutz? ?type-in-crosscuts? if there is a

continuous drop of hemoglobin and possible transfer for emergency endoscopy.

I can't catch the first word in Plan #6. The first sylable is a long i

sound. Any idea of what this might be? At first I thought maybe a

medication of some sort but now I don't think so. But if not a medicine,

what else? The dictator says this all in one breath. He is ESL but usually

speaks impressively clear.
