If you are in constant severe pain, you need more than you are getting. If your primary doc is not willing to address this, I sure would see someone else. If they got to spend one day in your physical body I bet they would sure change their tune about pain meds. I hate it that you have been so sick and are hurting so bad. I wish I could do something for you, you will be in my prayers.
