Hope this helps, Bets ! 8^)...... It is through the state...BUT my SURGEON had to request it. When I was in the hospital I requested a social worker come to visit me cause I knew...and my doctor knew i would not be able to do ANYTHING for AWHILE> My primary care doctor is in a hospital setting which makes it easier to continue the paper work each year in order to keep my help. ORIGINALLY I had a home attendant...which is with you almost everyday. If you need to find out how things work in your state..the best thing to do is to look for a STATE ADVOCATE FOR THE DISABLED. they can lead you to the proper agencies. I am on Medicare but also have something called a spend down program for Medicaid since they give more for home help than medicare. BECAUSE I am slightly over the income line...I have to pay...each month to them...what I am over. In Colorado...I would get it free. So you see it varies from state to state. The city/state will give you the forms and sometimes have a social worker come to your house to do so. Then your MD has to fill out most ot that form. Now if one does not have either of the above insurances...some states have other programs that basically do the same thing. So first look up the Disability state advocate...see what your city/state has. You can get the phone number from your Mayors or Congresspersons office.
