The university I work at has an office for students with disabilities.

A student should contact such a group upon admission and find out what

kind of documentation they need from your doctor in order to provide

you with services. Then, if you need a note taker (for example), they

ask another student in each class to take notes on two-part paper

(which they supply) so that you can get the " carbon " copy. If you need

special help to take exams, they can arrange for you to take them with

an appropriate proctor and other help. The documentation from your

doctor should describe the kinds of services you will need (oral

exams, note taking, etc.). One benefit to registering with the student

disability office at my university is that you get to sign up for

courses during " priority " registration (that is, ahead of all the

other students).
