The facility doing the test.  We don't do PA's for radiology; the facility is the one who wants to get paid, so they do the PA. Pratt

 

does anybody know what this means...

" Most prior auths (for radiology) are done by the radiology firms. "

I have to do prior authorizations and sit on hold for any MRI/CT.

If I can find a " radiology firm " I would prefer that.

Timely topic... and one of the most annoying.We send them patients, they make the money, and they want us to spend our time or pay a staff member to fill out form that help them make money. Ridiculous.

I push back. For the forms, I tell that them that I'm cash only and not contracted with the patients insurance company and don't have the info -- please get it themselves from the patient. :~)This am I got a call from a radiology ctr asking for a prior auth... I explained the above to her -- that this pt is in another state, and I have no contract with her insurance. If they can 't authorize the procedure, please provide me with the info I need to do it.. I try and put it back in their lap.

All ears to other ideas to make referrals less a time suck....???Anne

Yes, the radiology expects the PCP to get the prior auth prior to scheduling an appt.As far as not using the referral from of the specialist....well since I am usually begging them to see my patient in the first place.......for the most part they have so many patients that they do not budge on anything....and dealing with the office girls takes its own strategy!DannielleSubject: Re: prior auths for radiologyTo: Date: Monday, May 21, 2012, 11:21 PM

This amazes me (radiology facility obtaining prior auths)! Here (upstate NY) the first thing the scheduling person asks after you request a CT or MRI is "do you have the prior auth number?" If not, they explain that "you will need to get that and call us back." I'm afraid it makes sense to me, also, since I am the one that has the clinical info that justifies payment for the test. ---Sharlene

I have to do prior authorizations and sit on hold for any MRI/CT.If I can find a " radiology firm " I would prefer that.thanks,MaggieI thought it meant the " radiology firms " that the insurance companies sub-contract out the prior auth process to (in Southern Cal, for example, Blue Cross uses AIM).  Doesn't shift who is doing the work (primary care generally here although on a few things like Breast MRI the radiology facility will get the prior auth) to get the auth, but it is is a separate company the insurance company pays to process it.  Generally I have found them easier to use because they seem to have procotols and rules I can learn rather than randomness; not to mention online portals rather than waiting on hold and talking to someone maybe with a high school diploma and no medical training (much easier on my patience).  But still a hassle.

In NJ, specialists can not issue referrals

to themselves. The radiologists can not

get their own prior auths. Care Core and others don’t want to hear from

them. That removes the checks and balances. I order the test, I feel it’s necessary. I am contracted with the insurance, then it is up to me to do the referral or get the prior

auth. And, no, I haven’t found a way to

make it suck less.

What pisses me off is when specialists offices ask us to prior auth a procedure that they

are going to do. We fight back and forth

with the poor patient in the middle just worried about getting their test

done. Radiologic procedures is

this gray area. If I

order the needle bx, who gets the prior auth….me? or the radiologists

as I don’t know the procedure codes, etc.

Where I am, CT and MRI are competetive. All the facilities are willing to obtain the pre-auth. If they don't, I refer to the one which is willing to do it. And they all do. Think about it: It is big margin for them! Once in a while, they may ask my progress note. Rarely, the insurance doctors need to talk to me. That is after they get all the info, just to clarify: like they won't approve brain MRI and MRA unless MRI is abnormal, etc.

 Maybe call around and find which facility is willing to do it for your patients? I hate pre-auth, tests or meds. Tons of mine or assistant's time.  Wen San Mateo, CA

Same here, so many places to send for CT or MRI they generally handle the prior auth for me. Once in every 10 I may have an insurance I need to do the prior auth and basically give the note for the reason, o/w usually just order and they take care of all the details. Shop around, if there is some one who will do the prior auth for you, send all to them, the others will get the message!

similar to othershave all referring docs in EMR with generic template that says up to 20 visits and 1 year expiration - i write detailed note and then automatically attached pertinent patient summary info as well as scans of studies/labs and easily fax from EMR - only have 1 HMO (like 3 patients) that require quick online submission.  If the docs office doesnt like or accept my form, they simply will not get to see my patient though in reality that has never come up.  Even for mcare patients or other insurnaces that dont require referrals, I still send one b/c I want the specialist to have all the information (not that they read it anyways but that is a whole different story).

Have gotten local private imaging center to do my preauths even though they " are not allowed per insurance. "  Hospital also recently started similar program for some physicians after they realized they were losing biz by not doing preauths.  When I send over a radiology order to either place, I generally include enough clinical information on the order for them to handle a preauth.  Once in a while , I have to call to give some additional doc-to-doc info but < once monthly.

Only need to do drug preauth about once monthly.  Almost never used branded drugs.  Only preauths I find myself doing are PPIs once in a while and some sleeping meds for the rare patient I have taking them chronically.  Cant think of another med I've done preauth on recently.

Basically with my practice style, I rarely refer to start, don't order lots of imaging studies and only use generic meds (and generally use less meds than average internist) so while utterly annoying while I have to do things like that it really does not take up much time at all

I've only been told by the large university hospital I have to use their forms so I refuse to refer to their group unless I can't get a particular service anywhere else in the state (eg pediatric sleep study)  

As regards prior authsBCBS started these about 2 years ago for CT/MRIthey contracted with  medsolutions  a radiology pre auth firm to run the showall the hospitals and Xrays places would do them for us so I would just fax note and order to the company

then bcbs refused to let anyone do them except the doctor's office so I am back to doing themI try to do them during visit with patient present if I have enough time, i do use that time to continue during the visit. 

if not - do when I am writing long-winded posts to the group  :- ) or other work, never without something else running / doing in the back ground or I get too mad wasting timethe website for PA is not bad but I get stuck about 2/3 time ordering wrong procedure code and then have to call anyway to fix it

BCBS gave us the same song and dance, but the imaging businesses who want our business are more than willing to still do it if we send the chart note. BC does not really know is calling. The biggest pain is that many of the procedures that we think need

prior auth don't, and it is a real pain to call them and have them tell us that after several minutes on hold!

Maybe you should meet the particular specialists that you think do a good job. Ask for their help and make a deal that you will send them all of your referals if they pave the way. They may be " too busay " but I bet it is the front desk that thinks that,

not the doc.

As far as not using the referral from of the specialist....well since I am usually begging them to see my patient in the first place.......for the most part they have so many patients that they do not budge on anything....and dealing with the office girls takes

its own strategy!

This amazes me (radiology facility obtaining prior auths)! Here (upstate NY) the first thing the scheduling person asks after you request a CT or MRI is " do you have the prior auth number? " If not, they explain that " you will need to get that and call us

back. " I'm afraid it makes sense to me, also, since I am the one that has the clinical info that justifies payment for the test. ---Sharlene

The person who makes the money does the prior auth, that is my philosophy, and I don't use " their " forms. I have no issue calling up the docs and telling them, it is usually some office manager schtick.

Timely topic... and one of the most annoying.

We send them patients, they make the money, and they want us to spend our time or pay a staff member to fill out form that help them make money. Ridiculous.

I push back. For the forms, I tell that them that I'm cash only and not contracted with the patients insurance company and don't have the info -- please get it themselves from the patient. :~)

This am I got a call from a radiology ctr asking for a prior auth... I explained the above to her -- that this pt is in another state, and I have no contract with her insurance. If they can 't authorize the procedure, please provide me with the info I need

to do it.. I try and put it back in their lap.

All ears to other ideas to make referrals less a time suck....???

No, certain subsets of BC do not need prior auths in Colo, plan dependent. The hosptial/imaging centers, nor us, has any idea which ones until you call. Then when you get to the nurse on the phone they let you know that this plan does not require one.

So far we have convinced every imaging center to do the work.

BCBS gave us the same song and dance, but the imaging businesses who want our business are more than willing to still do it if we send the chart note. BC does not really know is calling. The biggest pain is that many of the procedures that we think need

I've only been told by the large university hospital I have to use their forms so I refuse to refer to their group unless I can't get a particular service anywhere else in the state (eg pediatric sleep study)

As regards prior auths

BCBS started these about 2 years ago for CT/MRI

they contracted with medsolutions a radiology pre auth firm to run the show

all the hospitals and Xrays places would do them for us so I would just fax note and order to the company

then bcbs refused to let anyone do them except the doctor's office so I am back to doing them

I try to do them during visit with patient present if I have enough time, i do use that time to continue during the visit.

if not - do when I am writing long-winded posts to the group :- )

or other work, never without something else running / doing in the back ground or I get too mad wasting time

the website for PA is not bad but I get stuck about 2/3 time ordering wrong procedure code and then have to call anyway to fix it
