I spend an hour and half with patient and family to hammer out DNR and Hospice and why the cancer treatment is not working, etc. or same amount of time trying to get an elderly woman away from her abuser but she's an adult and competent but I cannot get paid for my time. Now I submit nothing but level 5's and eat the rest. Sent the appeal 3 times with the notes and still get rejected. I quess they would rather pay for her traumatic brain injury the next time he "accidently swings a cabinet door" or "kicks the dog sensless". (so far I've been successful in getting 3 victims to leave their abusers -- out of 9 in the practice)These are all straight Medicare. I gave up on Advantage Programs long time ago.
