Craig, What limitations have you encountered with Practice Fusion? I would like to hear some case stories from offices using Practice Fusion in a primary care seeing 25 to 30 patients a day and still planning to make it their long term EMR. Know of anyone like this interested in sharing their story? I only see half that number of patients but the limitations that I currently notice are as follows; no ability to directly message/email patients from within the system, no ability to list current medications on the subjective portion of the note unless they have all been hand typed into a previous note, the template are extrememly easy to build but cannot share between templates...what I mean by this is that if I set up "CV: RR, no murmur/rubs or gallops" in the well woman exam i will have to create it again seperately in the URI template, few labs companies download automatically, only one lab currently accepts online orders straight from inside the program, I do not like that my nurse/MA cannot sign her own notes and then have it come to me for a co-sig.

HOWEVER....I had a system in the past with ALL of these bells and whistle and it cost me $15-18K in maintainenece alone the first year PF has cost me nothing!!!!! Also they are working very hard to add features as fast as possible. I love PF!!!

No financial interest...unless you count how much $$$ I have saved.

Thanks for the feedback.  Hope others will post their experience with PF in a solo office seeing perhaps 25 to 30 patients a day. My cost run $200/month for software support and now average $150 a month for IT support.  That gets me EMR, integrated billing, integrated labs, integrated faxing, etc.  Overall cost runs about 1.5% of revenue.  I bought 3 year old PCs three years ago to cut cost and they just chug along fine running XP.  At the time I got a couple extra at $100 each in case I needed spares.  Wonder how long they will last?  Maybe they will be collector items by the time I give them up.  Thanks again for the PF story. Craig, The big question with PF is internet availability in your area. Many seem to use PF and report no problem but many others have invested time and had to abandon it. In my area, which is supposed to have one of the best internet services in the country PF would not be workable. With three internet providers to choose from, none provide service availability that meets my needs. While many feel like they have great internet service, it’s not until you use it all day long every day that deficiencies become annoying. Perhaps someone knows of a utility that can run unattended over a month and produces a report showing the true internet availability. At today’s internet speeds in most areas of the country, there are many EMR applications that will never run as fast over the internet as they can from an office based server. In a low volume practice this may make little difference. When a second or two delay has happened the ten thousandth time it becomes annoying. When getting started the cost of an EMR may seem formidable. As your practice grows the relative cost become quite small. I can appreciate the dilemma of making long term investments when getting started. Every EMR choice is a bet on the viability of the vendor to hang in there for the long run, remain profitable, continue to innovate, etc. What PF is doing is interesting but not genuinely innovative since it’s little more than SaaS based on ads and reselling information to make it appear free. My guess is that at some point most other SaaS vendors (there are many) will jump in with “free products” that are more innovative. At that point, the challenge for survival of the fittest “free internet based EMR” begins in earnest. History tells us that in this upcoming internet innovation battle the winner will may be unpredictable. The likely consolidation of EMR vendors from some 400+ to a much smaller number will keep most of us pondering what to do and unfortunately most of us making the wrong long term choice. As this consolidation plays out, many will jump to the “free EMR model” in desperation. The forces of supply and demand will place enormous pressure on market leaders hoping to capitalize on generic offerings as the little guys willing to specialize whittle away their market share. Well, just like the rest of us, make sure you can download all of your data if the day comes that your vendor goes belly up. And, in the short run make sure you have the internet availability needed to be a happy PF user. All excellent points. Fortunately, I'm in a tech area where many businesses do their work in the cloud. However, I am easily annoyed by seconds of delay and unnecessary key strokes. I can see myself switching EMRs in the future. Stand by ....

Of note, PF is a relatively hot topic in the tech world. Several IT folks I talk to are familiar with it and their business plan. They are very excited about PF as the way for things to be done in the future. Not sure what that's worth, but there it is.
