I, Ken, wrote the request to improve my pediatric skills. I do not have to

be a peds specialist as yourself to work with children. Perhaps if

specialists as yourself would become more visible to the physician community,

your concern would be resolved. The sad fact of the matter is that there

are not that many PCS therapists in certain areas, and, in my case if there

are any, none are in home care. Thus, my request to refresh my skills is to

honor our profession and the Hippocratic Oath to " do no harm " . Hopefully any

therapist who encounters a client who is more complex than they can handle

would have the integrity to refer the patient to the physician requesting

someone more skilled (or certified) in the particular area. If you would

like some great ortho therapy courses to refresh your skills, I would be more

than happy to direct you. By the way, I am in the process of becoming an OCS

and would not be offended if you wrote such a request, with an " ortho twist " .

: I totally agree with you. When I first saw the original post, i made a

conscious decision not to answer right away but wait and see the discussion

because I did not want to be scathing!!! There are very specific competency

guidelines available from the Pediatric section tha twill help guide Ken in

developing his skills. It must be stated that if a child is getting PT via home

care the odds are it is a medically fragile, multiply complex case tha trequires

more than basic competencies. The PTs that see pediatric patients in the home

must be competent in dealing with vents, understand energy costs in a

developing, growing infant or child and have knowledge of other early

intervention resources in the community, and what is appropriate for home care.

Often the PT will be handling a number of issues that are beyond entry level .

If an Agency is serious about providing pediatric therapy services then

education and competency development must be provided to the practioners and

possibly consultative relationships established with experts in their area. I

speak from experience in providing home care services to pediatric patietns and

as serving as a consultant to other therapists.

One of the home care agencies I work for has asked me if I would do

manual therapy (in home care). I graduated from a PT school 20 years ago,

had a

good orthopedic knowledge base (Cyriax and I still have Dr. Cailliet's books)

and even did a clinical at 's Orthopedic clinic. I told them I could

probably " wing it " right now, but ethically, would rather refresh my manual

skills.

My questions are as such: Can anyone recommend a good refresher continuing

ed course, resource, or videotape? I can review my school notes, and can

take a " lecture-based " manual therapy course from a highly respected

instructor, but would like any input from those experienced in manual therapy

as to any good

courses/workshops for handling skills. Thanks.

I'm sorry if this twist on the question offends anyone and I truly believe

that Ken did not mean to offend. But I just could not let this go. My point

is: It is O.K. to refer to others when you need to. In the managed care

environment, I see physicians not referring patients to specialists because

of the financial consequences when it would be in the patient's best interest

to do so. I don't want that to happen to Physical therapist. Respect and

recognition of the specialities as well as the generalist is important to our

profession.
