So of 100 practices, what % have been in practice 1-3,4-7,8-10,10+ years. I believe this information would be valuable, as many of us have been "around a long time" yet others are "newbies." Different challenges for both groups! Right now I am doing locums work at an Indian Health Service site, so you can imagine just how much I truly want to be on my own and away from government hassles and ideals! Anyhow, I am a bit anxious about doing this, but I know that this will be the best thing for me to do. My practice is very small and very rural. I will be the only doctor in the Valley that holds about 2000 people. I practiced there for 7 years, so I have a lot of patients all ready. My wife is going to help me, and it will just be us two, a part time bookkeeper and an accountant. We are going to use Practice Fusion and it looks like Kareo (sp?) for billing. I am looking for advice, anything, please! I am not a business man, but I am a quick learner. More than that, I will do anything to get away from being micromanaged by the government and small town designated rural health clinics! Well, I think I am preaching to the choir here... I was told by so many people, mostly administrative folks, that the days of doctor hanging out his shingle and having a simple practice are through! I can't accept that. I have said it before, but you should look at Emds, seems expensive but it is integrated and it will save you bucket loads of money on the backend and is all set for meaningful use, so we will get our 18,000 this year. You can also expense the loan interest and depreciate the cost, so it is actually reasonable. Before I started new solo practice earlier this year, I use the list Serve as my To Do List. People in the group are also very helpful. One thing I would point out is about insurance. She trashed left and right my idea of opening my solo practice. She mentioned that the people in my town were moving out because of the economy. I have market analysis data that said the contrary and the same hospital just made a 60 million extra building next to it. She wanted me to join a bigger group, so I can practice medicine dictated by the board of directors. It is like buying a car and still hitch hike. I refuse to believe that being IMPish and solo is not a good idea. I found an office for 1000 per month ( first 3 months 500 so I can do all the health insurance paperwork). I went to many realtors in my town and they were showing me more money offices. Also all of them told me that was impossible to find a place so cheap and with no build out, guess what I found it , made business with the owner and no realtor commission . Also dr colas this America land of the small guys that can make it. I will have a meeting today with my local REC people , I want their input and see if help. I will get Emds, it is not the cheapest but it's what I want. I checked them since residency and I liked them. I like also amazing charts ( it looks cheap but u will spend money doing outsource billing) but it doesn't have billing included. I realize Dr Colas over the years after being stabbed in the back by white collars administrators ,that if I want to continue practice medicine with passion and loving it I must do solo IMP. Dirty little secret to share with hospital admins -- you don't want them to pay you ANYTHING so you are saving them money and MAKING them money when you send pts to them and their ancillaries. If you are the one paying, you don't need to ask permission. However, as I was quoted in my newspaper article http://www.post-gazette.com/pg/10276/1091955-28.stm , I love what I'm doing but I don't always pay myself each week. Each area is different, the admins don't look at things that way. You need to be careful though that you do not overextend yourself. Are you purchasing eMDs as a local server or a portal that you log into? Be sure if you use a portal that you can access your records if you don't renew your contract. Get help with things you don't know about. Post here with questions as most of us have confronted this before. Feel free to post to me directly or others of similar philosophy. Guest guest Posted August 18, 2011 Report Share Posted August 18, 2011 RE being solo. 1) as a good friend advisor told me "when you give it away for free there will be many takers" be cautious. 2) Consider NOT taking medicaid/medical assistance as you will work for very little -- cash discount prompt pay I wipe out 10% for this on day of care and you won't violate Medicare (take this even if low). 3) Take credit cards so you don't have to chase after bad checks.
