Where to buy a yellow fever stamp

Can anyone tell me where they bought thier Uniform Stamp for yellow fever. I

can't seem to find one that meets the dimensions (1 1/4 - 5/8).

I've ordered my address stamps (as well as envelopes, etc.) from:  iprint.com and am very happy.  Doesn't look like they have that size though (You could leave part of a bigger one blank; you can design the stamps with an online tool that let's you place everything.)

I looked for the vendor on the CA Yellow Fever certification page, but they don't list it. You could contact them though:  http://www.cdph.ca.gov/programs/immunize/Pages/CaliforniaYellowFeverVaccineProviderProgram.aspx

That would be nice, but in Indiana they give you the dimensions and what it needs to say and tell you to buy one from " the vendor of your choice. "

Last self inking stamp I ordered from https://www.customstampscheap.com/Was about $12 with shipping. Seems to be as good as others make.
