Other than medications, vitamins and diet are there reasons why someone's warfarin would not be working as well - lady on stable dose for years but cant get above 1.5 despite > 25% increase in dose to date

? Eating too much Vit k in MV or greens

Check the alcoholic beverages. I had one lady who was drinking a cranberry i (cocktail) daily. Once that was known, she switched drinks.
