Guest guest Posted April 8, 2011 Report Share Posted April 8, 2011 OK. I hate ER call and one particular ER doctor. Nuff said 53 yo old with no history presents with 2 weeks of unrelenting chestpain. Working, sleeping. Pain not aggravated by activity. Has not gone away. L side of chest radiating to back and discomfort in L arm of course. Sharp and stabbing 9 out of 10 level of pain. Not responding to ntg. Normal EKG and a CK that is 277 with normal MB and negative Troponin. Nurses tell me chest pain is reproducible with palpation and relieved somewhat by repositioning. ER doctor insists he needs to be admitted with serial enzymes to r/o MI as he has chest pain and “elevated cardiac enzymes”. Fam hx both parents had MIs but no idea at what age as ER doctor doesn’t bother to ask. Patient does not smoke. My opinion, patient has already ruled out as cardiac etiology as you don’t’ have a heart attack for 2 weeks without evidence of damage (or angina that is unrelenting). Am I wrong?
